Office Hours
To better serve our patients, we have expanded our clinical hours. This means that we sometimes see patients at times the business staff is not available. Telephone calls during this time will be directed to the answering service. We encourage you to call with any questions or concerns during business hours, when a qualified staff member will be able to offer assistance. Please note that we do not use a voice mail system – a real person answers all calls promptly.

Patients are seen by appointment and every effort is made to stay on schedule. Please plan to arrive a few minutes before your scheduled appointment to allow for information updates.

When the office is closed
In the case of life-threatening illness, please go immediately to the nearest emergency department. Call 911 if needed. The hospital will contact us.

When the office is closed, the telephone is answered by an excellent, well-trained answering service. Routine messages are retrieved when the office opens. Dr. Bergwerk is available in case of emergency and will return your call if necessary.

Insurance Plans
We care about your health care costs and try to limit your out of pocket expenses. We participate in many major insurance plans. Please do not rely on published lists to determine if this office participates in your specific program; call us to ask. Even if we do not participate in your plan, we would be delighted to see you. You will be responsible for payment at the time of your visit and we will provide you with necessary paperwork so that you may file your own claim.

As of November 2006 we will not be accepting new Medicare patients. Current patients who age into Medicare will be accommodated. We may be able to offer episodic care to visiting relatives of current patients.

Please present your insurance card at every visit. This will help us file the claim correctly. By contract, co-pays must be paid at the time of service.

Test Results
It is our practice to inform patients of all test results. Non-urgent results are mailed to the patient. Please call the office if you do not receive your test results within two weeks.

Prescription Renewals
Prescriptions are written with sufficient quantity to last until the next appointment. Please keep track of your medication and call for your next appointment before you run out of medication.

All prescriptions are transmitted electronically; there are no paper prescriptions. The prescriptions will be sent to the pharmacy of your choice, including mail-order plans.

Prescriptions will be written and electronically transmitted during office hours only.

Hospital Services
Dr. Bergwerk is on staff at the Norwalk Hospital. Should you require inpatient care, we will coordinate your care with the Norwalk Hospital hospitalist physician team. This office has secure remote access to your Norwalk Hospital inpatient chart.

Medical Records
We take medical confidentiality very seriously. Records are only released with your written authorization. A copy of your medical record can be sent to the physician of your choice. There is a copying fee that is set by state law. For your protection, records are sent by mail only. Dr. Bergwerk is always available to discuss care with a consulting physician.

Please let us know if a consulting physician needs specific information, such as laboratory results or electrocardiograms.  We can expedite the transfer of relevant important data.

There are special requirements for Insurance/Underwriting APS. Please call during office hours. Completed requests will be dispatched within ten business days. Fax requests will not be honored for an APS.

We would be happy to review your past medical records. Please do not have them faxed to the office, since they can be difficult to read. Illegible records are promptly shredded. Mail is received at the post office box only: please arrange to have your old records sent to:
